Personalize forms with URL parameters!

Dynamic variables in the forms!

More importantly, have the dynamic variables pulled in from URL parameters, which we could add to links to personalize forms and/or pre-fill them.

This way we can not only greet the user, like "Hey, Scott!" on the first page of the form, but we can also pre-fill the form with their email and/or other custom variables (whatever's available, i.e. a custom offer, like "$10 off your next purchase) to reduce the friction to complete the submission. And also make it feel like we're only emailing them!
